2. Aims and Objectives

- To provide high-quality, safe and accessible essential services that meet the reasonable needs of our registered patients.

- To support terminally ill patients in alignment with the Gold Standards Framework and their individual wishes.

- To optimise quality of life for patients with long-term conditions through proactive, personalised management.

- To deliver care that reflects patient choice, uses evidence-based practice, and involves the patient in shared decision-making.

- To ensure continuity of care by liaising effectively with other health and social care services.

- To deliver additional and enhanced services such as vaccinations, minor surgery, family planning, and chronic disease management.

- To meet Quality and Outcomes Framework (QOF) indicators and deliver measurable, improving standards of care.

- To manage patient information lawfully and respectfully in line with Information Governance requirements.

- To maintain a skilled, motivated and professional workforce through regular training and appraisal.

- To promote health and wellbeing through patient education, involvement, and engagement including our Patient Participation Group (PPG).

- To ensure continuous improvement through patient feedback and quality monitoring systems.

Essential Services:

- GP and nurse consultations

- Diagnosis and management of acute and chronic illnesses

- End of life care

- Maternity services (excluding delivery)

- Contraceptive services

- Routine vaccinations and immunisations

Enhanced Services:

- Minor surgery (e.g. joint injections, implants)

- Flu and pneumococcal immunisations

- 24-hour blood pressure monitoring

- Near patient testing (e.g. warfarin, diabetes)

- Patient participation activities

- Wound dressings

5. Location and Facilities

Following the destruction of the original building by fire in July 2017, Phoenix Family Practice has operated from a purpose-built portacabin facility at Weybridge Primary Care Centre since December 2017. In 2024, Church Street Practice and Rowan Tree Practice merged to become Phoenix Family Practice, and the merged practice now solely occupies the premises.

- The facility includes 8 consulting rooms, 8 treatment/nurse rooms, and administrative offices.
- It is fully accessible to patients with mobility and sensory needs and includes a shared waiting area and dedicated reception.
- Free car parking is available on site.
- The practice boundary covers Weybridge and surrounding KT13 areas.
